Who is the service for?

Referral Criteria

Update January 2025: Due to current capacity, we are only open for referrals from the West Leeds Transformation Hub only. We will update further once we are fully open to referrals again.

The Community Wellbeing Connectors enable people to increase their independence and support them to recover and improve their mental health and wellbeing. They engage with individuals in their own community settings, build trusting relationships, provide one-to-one outreach care and support. Supporting each individual to identify and engage with activities, groups and resources in their local communities to build their personal resilience and a personalised local support network.

The Community Wellbeing Connectors are part of a team located in three initial sites as part of the Community Mental Health Transformation for Leeds. This is a partnership of NHS organisations, Voluntary and Community Sector agencies and Leeds City Council Adult Social Care. The team work closely with people who have lived experience to transform how primary and community mental health services for people experiencing severe mental health issues are organised and delivered.

"The CWC worker helped me to get back into the gym, she supported me through things that I couldn't cope with like helping me sort my bills out and supporting me through my sons passing which she didn't have to do, but she listened to me then passed me onto the bereavement counselling which helped me. I never had that much support and her visits will be missed so thank you for everything most appreciated."
