Connect Together - Children and Young People Social Prescribing

Connect Together is a social prescribing service that supports children, young people and their families to make long lasting connections in their community.

Who is the service for?

How to access

Connect Together is for anybody aged 5 to 16. We have a number of ways in which young people can access the service:

  • Children and young people registered with the Armley Primary Care Network GP Surgeries (Armley Medical Practice, Priory View Medical Centre and Thornton Medical Centre)
  • Children and young people registered with the Beeston / Middleton and Hunslet Primary Care Network GP Surgeries (Shafton Lane Surgery, Parkside Surgery, Oakley Medical Practice ,Beeston Village Surgery ,City View Medical Practice, Leeds City Medical practice ,Church Street Medical Centre, Arthington Medical Centre, South Bank Surgery and Lingwell Croft Surgery)
  • Children and young people living within postcode areas LS12, 13 and 28

Making a Referral

Referrals can be made using the referral form below. If you require further information please contact CYP Referrals: cypreferrals@barca-leeds.org

You can be introduced to the service by your doctor, nurse or any other health care professional.

We welcome referrals from education settings and family support services.

We will work closely with Linking Leeds so young people aged 16 to 19 can access the social prescribing service that can best meet their needs rather than based on age criteria.

Connect Together is a social prescribing service that supports children, young people and their families to make long lasting connections in their community to bring about sustainable change and improvements to their lives.

How we can help

Some interventions may include:

  • Providing emotional and wellbeing support
  • Develop physical activities for children and facilitate the use of outdoor spaces
  • Organise creative activities to fill community gaps
  • Addressing weight management
  • Helping with school readiness
  • Supporting social and emotional skills
  • Encouraging positive community engagement and fostering community links
