The Health and Wellbeing Navigators work with people who are rough sleeping, vulnerably housed and may be living street-based lives. These individuals have a number of physical and mental health comorbidities and are at high risk of severe health complications. There is increased vulnerability to exploitation and harm which is particularly relevant for those who are rough sleeping, women, asylum seekers and refugees, and those who have no recourse to public funds. We work with people who have experienced a lot of adversity in their lives, including homelessness, substance misuse, mental health issues and may have a history of offending behaviour and can find it difficult to engage in services. Through co-production we will speak to people who we are supporting to understand their health needs and consider how we can support them to access the care they need.
Referral Criteria
This service is not open to general referrals. It will work with people who are already known to the Barca-Leeds' Housing Navigators and Simon on the Streets and will benefit from intensive support for their health and wellbeing needs.
The Health and Wellbeing Navigators work with people who are experiencing homelessness, supporting them to access the right health services and care to meet their needs.
This partnership between Barca-Leeds and Simon on the Streets supports people to overcome the barriers in accessing health and wellbeing services, and work towards the person’s individual goals in relation to improving their health. Our Health and Wellbeing Navigators work across Leeds as part of a team, working closely with Barca-Leeds Housing Navigators, Barca Outreach Support Team and Simon on the Streets Outreach Workers.
